Job summary

Pathways Healthcare is seeking a dedicated Registered Nurse to provide in-home care in the Concord/ Lowell area under a per diem arrangement. You will assess patients, develop plans of care, administer medications, and collaborate with physicians and care teams to ensure high-quality outcomes.

The role requires an active RN license in Massachusetts, 2+ years in geriatrics preferred, strong communication, and a valid driver's license with access to a vehicle.

Qualifications

• Active RN license in the state where the position is based.
• Preferably 2+ years of nursing experience with a geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
