Job summary

Pathways Healthcare in Manchester, NH is seeking an experienced registered nurse for full-time, Monday–Friday in-home patient care. You will assess patients, develop and implement care plans, administer medications, and collaborate with physicians to ensure high-quality outcomes.

Applicants should have 2+ years of nursing experience (geriatrics preferred), a nursing degree, valid BLS/CPR, and an active driver's license.

Qualifications

2+ years nursing experience, preferably geriatrics.
Degree in Nursing from an accredited institution.
Proficient with electronic medical records.
Excellent bedside manner and communication skills.
Working knowledge of health care laws and regulations.
Maintain current BLS CPR certification.
Possess a valid driver's license and own a reliable vehicle.

Responsibilities

Assesses and evaluates patient's status; writes and initiates plan of care.
Regularly re-evaluating patient and family/caregiver needs.
Revising the plan of care as necessary.
Develops a care plan with goals based on nursing diagnosis.
Initiates preventive and rehabilitative nursing procedures; administers medications.
Documents and reports patient condition and care given.
Communicates with physician regarding patient needs and changes.
Maintains a safe patient environment and attends meetings.
Participates in on-call duties and coordinates equipment/services.
Supervises ancillary personnel and delegates as needed.
