Humana Healthy Horizons in Virginia is seeking an experienced RN to serve as Field Care Manager in Chester, VA. You will perform telephonic and face-to-face assessments for LTSS and ABD populations, develop individualized care plans, and guide members toward appropriate resources.

This full-time role requires an active Virginia RN license, 2 years in healthcare/case management, and 1 year with high-risk populations. Expect travel 50–75% and a Monday–Friday schedule with EST hours.

About This Role

Humana Healthy Horizons in Virginia is seeking RN, Field Care Managers to perform telephonic and face-to-face assessments for long-term services and support (LTSS) and aged, blind, and disabled (ABD) member populations. This role is ideal for experienced nurses who can work independently and make autonomous decisions while providing care and support to members.

Highlights

• **Pay:** $71,100 - $97,800/year
• **Location:** Chester, VA
• **Residency:** Must reside in the Central Region of Virginia (Richmond, Henrico, or Midlothian areas)
• **Schedule:** Monday – Friday; 8:0am – 5:00pm EST
• **Travel:** 50 - 75% field interactions with members and families

Requirements

• Active RN license in Virginia without disciplinary action required.
