Job summary

Concierge Home Care is seeking an experienced Registered Nurse (RN) Case Manager to provide in-home clinical oversight in Monroe, NJ. This field-based role supports clients with personalized care plans and ongoing clinical supervision, including supervision of Home Health Aides and Nurses.

The ideal candidate will have an active NJ RN license, 2+ years in acute care or home care, and experience with vents, tracheostomies, G-tube/PEG, and IV infusions.

Qualifications

• Active New Jersey RN license required.
• Minimum 2 years of RN experience in acute care, home care, or case management.
• Experience with ventilator and tracheostomy care.
• Experience with G-tube/PEG tube management.
• Experience with IV infusions.
• Wound care experience.
• Current BLS Certification.
• Current physical examination within the past year.
• PPD or QuantiFERON test within the past year, or chest X-ray within the past five years if applicable.
• MMR immunization documentation.
• Valid driver's license and reliable transportation.
• Authorized to work in the United States.

Responsibilities

• Conduct comprehensive in-home nursing assessments.
• Develop, implement, and oversee individualized care plans.
• Obtain and maintain physician orders in compliance with New Jersey regulations.
• Monitor client conditions and communicate significant changes.
• Complete accurate and timely clinical documentation.
• Perform Home Health Aide supervisory visits and ensure quality of care.
• Provide clinical guidance and support to Home Health Aides and Nurses.
• Promote client safety, independence, comfort, and overall well-being.
