Job summary

Concierge Home Care is seeking an experienced Registered Nurse (RN) Case Manager to provide in-home clinical oversight. This field-based role requires organization, independence, and a passion for building meaningful relationships with clients while delivering high-quality care in their homes.

Responsibilities include comprehensive in-home assessments, care planning, obtaining physician orders per NJ regulations, and supervising Home Health Aides.

Qualifications

• Active New Jersey RN license.
• Minimum 2 years RN experience in acute care, home care, or case management.
• Current BLS Certification.
• Current physical examination within the past year.
• Current PPD or QuantiFERON test within the past year, or chest X-ray within past five years, if applicable.
• MMR immunization documentation.
• Valid driver’s license and reliable transportation.
• Authorized to work in the United States.

• Recent experience with:
  • Ventilator and tracheostomy care
  • G-tube/PEG tube management
  • IV infusions
  • Wound care
• Current BLS Certification.
• Current physical examination (within the past year).
• Current PPD or QuantiFERON test (within the past year), or chest X-ray completed within the past five years, if applicable.
• MMR immunization documentation.
• Valid driver’s license and reliable transportation.
• Authorized to work in the United States.

Compensation

• RN Assessment: $150 per visit
• RN Aide Supervision: $125 per visit
• Hourly Clinical Visits: $55 per hour
• Weekday On-Call: $50 per day (Monday-Friday, 5:00 PM-9:00 AM)
• Weekend On-Call: $110 per day (Saturday 8:00 AM-Monday 9:00 AM)
