Professional Case Management is seeking RN-Case Managers in Kentucky to provide in-home nursing and comprehensive case management for former Nuclear Weapons Workers with chronic/terminal illnesses. You will perform weekly nursing visits, conduct vital assessments, ensure home safety, and oversee caregivers.

Qualifikationen

• RN licensure in the state(s) of practice.
• BSN is preferred.
• Minimum of two years nursing experience, including at least one year in home care or closely related field.
• One year of supervisory or case management experience is preferred.
• Current CPR certification.

Aufgaben

• Provide direct care and case management for assigned clients.
• Plan, implement, and evaluate care in coordination with the regional director.
• Assist with staffing and scheduling within scope of practice and client needs.
• Collaborate with personnel to identify and improve services and perform targeted performance improvements.
• Maintain accurate clinical documentation and participate in agency performance initiatives.
