Job summary

Professional Case Management is seeking an in-home Registered Nurse (RN) Case Manager in Paducah, KY. You will conduct weekly in-home nursing visits, perform vitals assessments, ensure home safety, and manage weekly medication planners for former nuclear workers with chronic/terminal illnesses.

The role requires an active RN license, 2+ years nursing experience (1 year in home care), and CPR certification. Part-time hours with pay at $42/hour, benefits, and supportive supervision are provided.

Qualifications

Graduate of a state approved school of professional registered nursing.
Current, unrestricted RN license in the state(s) of practice.
Minimum of two years nursing experience including one year in home care or closely related field.
One year of supervisory and/or case management experience preferred.
Current CPR certification.

Responsibilities

Provide direct care and case management of assigned clients.
Plan, implement, and evaluate care provided.
Coordinate and manage client care conferences as needed.
Assist staffing coordinators regarding staffing and scheduling within scope of practice.
