Job summary

Professional Case Management in Kentucky is hiring RN Case Managers to provide in-home nursing visits and manage client care for former Nuclear Weapons Workers with chronic illnesses. You’ll assess, plan, andCoordinate care on a weekly basis, with drive time paid and a flexible schedule that supports personal commitments.

The role emphasizes one-on-one patient relationships, adherence to agency policies, and collaboration with the regional director to ensure high-quality care.

Qualifications

• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• CPR certification is required.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with the regional director and staff to improve services.
• Plan, implement, and evaluate care provided; manage client care conferences as needed.
• Serve as a local on-site clinical resource to ensure client’s home care needs are met.
• Coordinate staffing and scheduling with personnel within scope of practice and client needs.
• Adhere to nursing delegation guidelines and agency policies.
• Participate in interviewing, selection, and evaluation of clinical personnel as requested.
