Qualifications

• Graduate of a state approved school of professional registered nursing.
• Current, unrestricted RN license in the state of practice.
• Minimum of two years nursing experience with one year in home care.
• One year supervisory/case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management for assigned clients.
• Plan, implement, and evaluate care in collaboration with regional leadership.
• Coordinate client care conferences and document outcomes.
• Support staffing decisions and ensure scope of practice adherence.
• Complete clinical documentation and participate in quality initiatives.
