Job summary

Professional Case Management (PCM) is seeking RN-Case Managers to perform in-home nursing visits for former nuclear weapons workers in Ohio. You will assess health status, develop care plans, and supervise caregivers, with visits typically lasting 1–2 hours weekly.

The role offers a $42.00 hourly rate with weekly pay and drive time included, enabling you to create your own schedule while delivering compassionate, patient‑centered home care and fostering strong nurse–patient relationships.

Qualifications

• CPR certification is required.
• Two years nursing experience including one year in home care or closely related field.
• One year of supervisory and/or case management experience preferred.
• Current, unrestricted RN license in the state of practice.

Responsibilities

• Provide direct care and case management for assigned clients.
• Plan, implement, and evaluate home care services.
• Coordinate with regional director and staff to identify and improve services.
• Lead client care conferences and document care plans.
• Ensure adherence to agency policies and nursing delegation guidelines.
