Professional Case Management (PCM) is hiring RN Case Managers in Tennessee to conduct in-home nursing visits for former nuclear weapons workers with chronic illnesses. You will assess, plan, and coordinate care on a weekly basis, ensuring home safety and medication management while supervising caregivers.

The role supports flexible scheduling, competitive hourly pay, and opportunities to balance work with personal commitments. CPR and RN licensure are required, with BSN preferred.

Qualifications

• Current, unrestricted RN license in the state of practice.
• Minimum two years nursing experience, including one year in home care.
• One year supervisory or case management experience preferred.
• CPR certification required.

Responsibilities

• Provide direct care and case management for assigned clients.
• Coordinate with regional director and staff to improve services.
• Conduct care planning, implementation and evaluation.
• Collaborate with staffing to ensure appropriate scheduling.
• Perform supervisory visits and ensure policy adherence.
