Job summary

Concierge Home Care is seeking an experienced Registered Nurse (RN) Case Manager for a field-based, per diem role in Monroe, NJ. You will conduct in-home assessments, develop care plans, and supervise Home Health Aides while coordinating with physicians.

A valid NJ RN license and 2+ years of relevant experience are required. The position offers flexible per diem scheduling with competitive compensation and a supportive clinical team.

Qualifications

• Active New Jersey RN license required.
• Minimum 2 years RN experience in acute care, home care, or case management.
• Experience with ventilator/tracheostomy care, G-tube/PEG, IVs, and wound care.
• Current BLS certification and ability to work in the United States.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive in-home nursing assessments.
• Develop and oversee individualized plans of care.
• Obtain physician orders per NJ regulations and monitor changes.
• Document clinical activities accurately and timely.
• Supervise Home Health Aides; guide nursing staff.
• Provide case management and clinical oversight in homes.
