Job summary

Visiting Angels seeks a Home Care Field RN/Case Manager for in-home visits across Mercer County and optionally Burlington County. This per diem, part-time role requires an NJ RN license, strong communication, and the ability to travel to clients’ homes. Mileage is reimbursed and visits are weekdays when possible.

The RN will perform initial assessments and care plan development, with reassessments and follow-ups to ensure high-quality, personalized care for older adults in their homes.

Qualifications

• Previous Home Care RN experience highly preferred.
• Current New Jersey RN license.
• Ideal candidate resides in Mercer County and travels to client homes.

Responsibilities

• Make appointments with clients in their Mercer County homes; mileage reimbursed.
• Create in-home initial assessments and care plans based on client needs.
• Conduct follow-up assessments at specified intervals.
• Perform periodic telephone assessments and revise care plans as needed.
• Supervise CHHAs and conduct Orientations/Skills Assessments.
