Job summary

FreedomCare is seeking a Registered Nurse (RN) Home Health Case Management for a nursing job in Fairfield, Connecticut. This is a staff, full-time role with a standard 40-hour work week and 8-hour days, focused on in-home assessments and ongoing care management.

You will conduct initial in-home assessments, develop and revise plans of care, coordinate with physicians and care teams, and ensure regulatory compliance while delivering person-centered care. New graduates are welcome to apply.

Qualifications

• Diploma from an accredited nursing program.
• New graduates welcome; home care case management is a plus.
• Care coordination experience required.
• Active RN license in the state of Connecticut.
• Must have a valid driver’s license and access to a car.
• Proficient with Case Management Systems and MS Office.

Responsibilities

• Conducts initial in-person clinical assessments and coordinates service activation.
• Develops, implements, and manages the patient’s plan of care; revise as needed.
• Coordinate care with physicians and interdisciplinary teams; document changes.
• Provide ongoing coaching to promote best practices and caregiver education.
• Review caregiver notes daily and address changes in the patient’s condition.
• Ensure continuous regulatory compliance and adherence to company policies.
• Perform other duties as required by the supervisor.
