Job summary

Phoenix Home Care and Hospice in Missouri is seeking a Home Health Case Manager to deliver hands-on patient care in patients' homes, coordinating services, communicating with insurers, and supervising visiting nurses.

You will work with a therapy team (PT/OT/Speech), conduct initial assessments, develop care plans aligned with orders, and ensure high quality, compassionate home-based care.

Qualifications

• Missour i- or Compact-licensed RN required.
• Valid driver’s license required.
• Vehicle with current auto insurance preferred.
• Home Health or Critical Care experience preferred.

Responsibilities

• Manage caseload including supervision of LPN visits.
• Handle varied weekly caseload with routine visits, evaluations, and starts of care.
• Educate patients and families on health conditions such as CHF, COPD, diabetes, wound care, and medication management.
• Communicate with physicians to obtain orders and write diagnosis-driven plans of care.
• Collaborate with PT/OT/Speech and other therapy teammates.
