Qualifications

• Missouri or Compact RN license is required.
• Driver's license and active auto insurance are required.
• Home Health or Critical Care experience preferred.

Responsibilities

• Manage caseload and supervise LPN visits.
• Coordinate routine visits, evaluations, and care starts.
• Educate patients on CHF, COPD, diabetes, wound care and meds.
• Communicate orders with physicians and write care plans.
• Collaborate with PT/OT/Speech therapy teams.
