Phoenix Home Care & Hospice in Kansas City, MO is seeking registered nurses to provide compassionate home health care across Kansas City and surrounding areas. You will manage a patient caseload, supervise LPN visits, and ensure high clinical standards and regulatory compliance.

This role emphasizes real-time charting, patient and family education on conditions like CHF, COPD, Diabetes, wound care, and medication management.

Qualifications

• Active Missouri or Compact RN license.
• Valid Missouri Driver's License.
• Home health experience preferred.
• Strong clinical skills in physical assessment and diagnostic interpretation.
• Compassionate communicator with patients, families, and physicians.

Responsabilités

• Manage your own caseload including supervision of LPN visits — you run your schedule.
• Varied weekly visits including routine care, evaluations, and starts of care.
• Educate patients on health conditions including CHF, COPD, Diabetes, wound care, and medication management.
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care.
• Work alongside a therapy team including PT, OT, and Speech.
