Job summary

Phoenix Home Care in Overland Park, KS is seeking a Home Health Case Manager to deliver hands-on patient care in clients' homes. You will coordinate services, communicate with health insurers, and supervise visiting nurses while developing personalized care plans aligned with physician orders.

You will collaborate with Nurses and Therapists, utilize Roger’s AI-powered support to streamline charting, and maintain a strong focus on work-life balance while providing compassionate, high-quality care.

Qualifications

• Active RN license in Kansas or a compact state.
• Experience in home health or critical care preferred.
• Ability to coordinate care across disciplines.

Responsibilities

• Management of caseload including supervision of LPN visits.
• Varied weekly caseload includes routine visits, evaluations, and starts of care.
• Education on CHF, COPD, Diabetes, Wound Care, and Medication Management.
• Communication with physicians to obtain orders and write plans of care.
• Collaborate with PT/OT/Speech therapy teams.
