Job summary

Phoenix Home Care and Hospice in Springfield, Missouri seeks a Home Health Case Manager to coordinate patient care at home, supervise LPN visits, and ensure care plans align with physician orders.

You will collaborate with physicians and a therapy team (PT/OT/Speech), educate families on conditions such as CHF, COPD, diabetes, and wound care, and communicate effectively to coordinate services.

Qualifications

• Missouri or Compact RN license required.
• Driver’s license and vehicle with current auto insurance.
• Home Health or Critical Care experience preferred.

Responsibilities

• Manage caseload including supervision of LPN visits.
• Conduct visits including routine assessments and starts of care.
• Educate patients and families on CHF, COPD, Diabetes, wound care, and meds.
• Communicate with physicians to obtain orders and write care plans.
• Collaborate with PT/OT/Speech therapy teams.
