Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license with the ability to practice in Minnesota.
• BLS certification.
• Demonstrated knowledge of safety and infection control practices.
• Ability to pass company background checks and drug screening.
• Valid driver's license with reliable transportation.

Responsibilities

• Perform comprehensive assessments of client status including physical, psychosocial, and environmental parameters.
• Develop an individualized plan of care for each client that incorporates data analysis and current findings.
• Provide interventions focused on achieving realistic client outcomes.
• Report changes to update the plan of care as progress is made.
• Maintain detailed client records showing assessments, planning, interventions, and evaluations.
• Initiate referrals to other health team members as needed.
