ComForCare Home Care is hiring a Registered Nurse (RN) who is ready to step from fast-paced clinical environments into a role where your expertise shapes home-based care.

You’ll serve as the clinical anchor for in-home services: assess patients, update care plans, supervise CNAs and HHAs, coordinate follow-up, and communicate with families and care teams to ensure safe, high-quality care delivery.

Qualifications

• Active Maryland RN license or compact MD eligibility.
• CPR/First Aid certification.
• Reliable transportation for home visits.
• Strong clinical assessment and communication skills.
• Experience in home care, hospice, or geriatrics preferred.

Responsibilities

• Conduct in-home patient assessments and ongoing evaluations.
• Develop, update, and oversee individualized care plans.
• Supervise and support CNAs, GNAs, HHAs, and Med Techs.
• Monitor changes in patient condition and coordinate follow-up care.
• Communicate with families, physicians, and care teams.
• Ensure safe, compliant, high-quality care delivery.
• Maintain accurate and timely clinical documentation.
• Provide guidance, education, and clinical support to caregivers.
