Job summary

Comfort Keepers of Marietta is seeking an RN Care Manager to design person-centered plans of care and champion quality that keeps seniors safe, healthy, and comfortable at home in Cobb County, Georgia.

You’ll assess, plan, monitor, and document in a HIPAA-secure EMR, collaborate with physicians and therapists, and set a flexible visit schedule with remote charting. Active Georgia RN license, 2+ years of geriatric/home-care experience, CPR/First Aid and a valid driver’s license are required.

Qualifications

• Active RN license in Georgia
• 2+ years’ geriatric/home-care experience
• Solid clinical assessment skills
• CPR/First Aid certification and a valid driver’s license

Responsibilities

• Assess & Plan: complete in-home RN assessments; craft individualized Care Plans with client/family approval.
• Monitor & Document: review daily care logs, track condition changes, and interventions in our HIPAA-secure EMR; elevate concerns promptly.
• Collaborate & Advocate: coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
