Job summary

Community LIFE Bedford in Bedford, PA, seeks a Home Care Coordinator (COTA/L) to assess participant needs and coordinate home-based services for seniors. You will work with the interdisciplinary team to maximize rehab potential and support transitions from hospital to home, on weekends and on-call shifts.

Ideal candidates hold a PA COTA license, have 1+ year experience with frail elderly populations, and can navigate health care networks while delivering compassionate, independent care.

Qualifications

• Graduate of accredited COTA program.
• At least 1 year experience with frail elderly populations.
• Experience teaching patients and other health workers preferred.

Responsibilities

• Assess participant needs and coordinate home/personal care services.
• Collaborate with interdisciplinary team to formulate care plans.
• Support transitions to and from home after hospitalization or skilled care.
• Participate in weekend and on-call rotation.
