Job summary

Apex Home Health, a CMS 5-Star rated home health provider serving Louisiana, seeks a Registered Nurse to deliver in-home patient care across the Northshore, New Orleans, and Baton Rouge areas.

You will perform skilled nursing visits, assess patient needs, develop care plans, administer medications, and document all care in EHRs, while collaborating with physicians and the care team to achieve optimal patient outcomes.

Qualifications

• Active, unrestricted RN license in Louisiana and CPR certification.
• Clinical skills in assessment, care planning, med administration, wound care, and chronic disease management.
• Strong documentation and organizational abilities, including EHR charting.
• Effective communication and collaboration with interdisciplinary teams.

Responsibilities

• Perform skilled nursing visits and assess patient conditions.
• Develop and implement individualized care plans and coordinate care.
• Administer medications and treatments and monitor outcomes.
• Document all care in EHR and ensure regulatory compliance.
• Educate patients and families on disease management and prevention.
• Participate in case conferences and quality improvement initiatives.
