Job summary

Ahhmgt is seeking a registered nurse for on-road home health visits in Louisiana. You will assess patient health problems, develop and implement care plans, and administer nursing care to ill or disabled patients.

You will coordinate with physicians, educate patients and families, and maintain medical records during field visits. Required are an active RN license, CPR certification, and 1–5 years of home health experience.

Qualifications

Responsibilities

• Monitor, record and report symptoms and changes in patients' conditions.
• Direct and supervise nursing or health care personnel.
• Coordinate with physicians to plan and evaluate patient care.
• Instruct patients and families on health education and disease prevention.
• Maintain accurate, detailed reports and records.
