Job summary

Mind & Mobility Home Care is seeking a compassionate Registered Nurse (RN) to provide in-home, private duty nursing in Collier County, FL. You will assess health needs, supervise caregivers, and collaborate with families to promote safety and wellbeing.

The ideal candidate will have an active Florida RN license, strong clinical judgment, and excellent communication skills, with travel up to 75% as needed for supervisory visits.

Qualifications

• Active RN license in the state of practice.
• Previous experience in home health, private duty, or outpatient nursing preferred.
• Strong clinical assessment and patient care skills.
• Excellent communication and interpersonal skills to work effectively with patients, families, and caregivers.

Responsibilities

• Assess patients' health status and needs in the home setting.
• Develop and implement individualized care plans in collaboration with patients, families, and caregivers.
• Educate patients and families on disease management, health maintenance, and safety.
• Communicate patient progress, concerns, and changes in condition to supervisors and other care team members.
• Maintain accurate and timely documentation of care in compliance with company policies and regulations.
• Promote a safe, comfortable, and supportive environment for patients in their homes.
• Conduct caregiver evaluations and assess caregivers' skills to ensure quality of care.
• Perform supervisory visits with caregivers to observe care delivery, provide guidance, and ensure compliance with care plans.
