Job summary

Mind & Mobility is seeking a compassionate Registered Nurse (RN) to provide in-home care in Hillsborough County, FL. The PRN role involves assessing patient health needs, supervising caregivers, and coordinating with families to promote safety and well-being.

The ideal candidate holds an active FL RN license, has home health or private duty experience, strong clinical judgment, and excellent communication. Travel up to 75% is required, with access to EMR systems and ongoing training.

Qualifications

• Active RN license in Florida.
• Experience in home health, private duty, or outpatient nursing preferred.
• Strong clinical assessment and patient care skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and a valid driver\'s license.
• EMR proficiency and basic computer skills.
• CPR and other relevant certifications as required.
• Willingness to travel up to 75% of the time.

Responsibilities

• Assess patients\' health status and needs in the home setting.
• Develop and implement individualized care plans with patients and families.
• Educate patients and families on disease management and safety.
• Communicate patient progress and changes to supervisors and team.
• Maintain accurate documentation in compliance with policies.
• Promote a safe, comfortable home environment.
• Conduct caregiver evaluations and supervise home visits.
• Perform supervisory visits to ensure care plan adherence.
