Job summary

Mind Mobility is seeking a compassionate Registered Nurse (RN) to provide in-home care for clients in the Palm Harbor, FL area. The role includes patient health assessments, care plan development, and supervision of caregivers to promote safety and well-being.

The position is PRN with travel up to 75% and requires an active Florida RN license, strong clinical judgment, and excellent communication skills. Competitive pay and flexible scheduling are offered.

Qualifications

• Active RN license in Florida required.
• Experience in home health or private duty preferred.
• Strong clinical assessment and patient care skills.
• Excellent communication and interpersonal skills for patients, families, and caregivers.
• Ability to work independently with a flexible schedule.
• Reliable transportation and valid driver\'s license.
• CPR and related certifications as required.
• Willingness to travel up to 75% of the time.

Responsibilities

• Assess patients\' health status in the home setting.
• Develop and implement individualized care plans with families and caregivers.
• Educate patients and families on disease management, health maintenance, and safety.
• Communicate patient progress to supervisors and care team.
• Maintain accurate documentation per company policies.
• Promote a safe, comfortable home environment for patients.
• Conduct caregiver evaluations to ensure care quality.
• Perform supervisory visits to observe care delivery and ensure plan adherence.
