Job summary

Mind Mobility in Brandon, FL is seeking a compassionate Registered Nurse (RN) to provide in-home care for clients in our private duty home health services. The RN will assess health needs, supervise caregivers, and collaborate with families to promote safety and well-being.

Qualifications

• Active RN license in the state of practice.
• Experience in home health, private duty, or outpatient nursing preferred.
• Strong clinical assessment and patient care skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and a valid driver's license.
• Proficiency with electronic medical records (EMR) and basic computer skills.
• CPR and other relevant certifications as required.
• Willingness to travel up to 75% of the time.

Responsibilities

• Assess patients' health status and needs in the home setting.
• Develop and implement individualized care plans in collaboration with patients, families, and caregivers.
• Educate patients and families on disease management, health maintenance, and safety.
• Communicate patient progress, concerns, and changes in condition to supervisors and other care team members.
• Maintain accurate and timely documentation of care in compliance with company policies and regulations.
• Promote a safe, comfortable, and supportive environment for patients in their homes.
• Conduct caregiver evaluations and assess caregivers' skills to ensure quality of care.
• Perform supervisory visits with caregivers to observe care delivery, provide guidance, and ensure compliance with care plans.
