As an RN, you will assess patients, educate families, assist with ADLs, coordinate with care teams, administer treatments, document care in our EMR, and provide emotional support—all while maintaining dignity and independence.

Qualifications

• Active RN license in the state of practice.
• Willingness to travel to patients' homes.
• Graduation from an accredited school of nursing.
• Current BLS certification.
• Ability to pass state and federal background checks.
• Professional liability is required.
• Strong problem-solving and empathetic approach.

Responsibilities

• Conduct thorough assessments and provide personalized home care.
• Educate patients and families on health maintenance and disease prevention.
• Assist with ADLs with dignity and respect.
• Coordinate schedules with patients according to their plan of care and preferred hours.
• Monitor changes in patient symptoms and report promptly.
• Administer medications and treatments as prescribed by the physician.
• Communicate with Case Managers and collaborating physicians to ensure unified care.
• Maintain accurate patient logs in the EMR reflecting the journey of care.
• Provide emotional support to patients and their families.
