Job summary

CHRISTUS Homecare, part of LHC Group and within the Optum family, seeks a Registered Nurse to provide and direct nursing care in patients' homes across Louisiana. You will coordinate the plan of care with physicians and care teams, ensuring compliance with regulations and agency policies while supporting families and patients in achieving optimal outcomes.

Responsibilities include clinical supervision, accurate documentation, and collaboration with the care team to maintain continuity of care.

Qualifications

• Current, unrestricted RN licensure in state of practice.
• Valid driver's license and access to reliable transportation.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with laws and agency policies.
• Coordinate total plan of care with other health care professionals and communicate with physicians, agency staff, patients and families.
• Perform supervisory visits for home health aides and licensed nurses as required by state/federal regulations.
