Job summary

Acadian HomeCare of New Iberia, part of LHC Group and the Optum family, is seeking a Registered Nurse for PRN home health visits in Louisiana. You will provide and direct nursing care in patients' homes as prescribed by physicians, coordinating with the care team to ensure continuity of care.

The role requires current RN licensure in Louisiana, a valid driver’s license, and CPR. Preferred: 1+ year RN experience.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, vehicle insurance and access to a dependable vehicle or public transportation.
• Preferred: Current CPR Certification or ability to complete within 90 days of hire.
• Preferred: 1+ years of RN experience.
• LA state-specific: RN licensure must have no restrictions and at least one year of clinical RN experience.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice.
• Coordinate care with other team members from admission to discharge.
• Complete clinical nursing assessments per program requirements.
• Ensure patient eligibility and medical necessity for services.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency and duration meets patient needs and obtain physician approval.
