Job summary

Centric Home Health and Hospice is seeking a compassionate Registered Nurse to deliver in-home nursing care across our Texas service area. You will coordinate patient care, perform skilled nursing procedures, and collaborate with an interdisciplinary team to achieve optimal outcomes.

The role requires a valid RN license, CPR certification, and at least 1 year of clinical nursing experience, preferably in home health.

Qualifications

• Graduate of an accredited diploma, associate, or bachelor nursing program.
• Current Registered Nurse (RN) state license.
• At least 1 year of clinical nursing experience; home health preferred.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Conduct initial and ongoing patient assessments.
• Develop, implement, and update individualized plans of care.
• Provide direct skilled nursing care as ordered by physicians.
• Educate patients and caregivers on health needs and procedures.
• Communicate changes in patient condition to providers and care team.
• Complete clinical documentation accurately and timely.
• Participate in on-call rotation as assigned.
• Ensure compliance with infection control and agency policies.
• Uphold agency policies and professional standards.
