Advanced Home Health and Hospice seeks an experienced Home Health Registered Nurse (RN) to provide patient-centered care in patients’ homes. You will administer medications, monitor conditions, and collaborate with a care team to develop tailored plans.

The role requires strong clinical skills, excellent communication, and a willingness to travel to patient homes to deliver compassionate, high-quality care in a home setting.

Qualificações

• Must hold a valid RN license in the state of work.
• Willingness to travel to patient homes.
• Wound care experience preferred.
• Strong clinical nursing skills and precision.

Responsabilidades

• Administer medications and treatments as prescribed by physicians and in accordance with home health care protocols.
• Perform routine and specialized nursing tasks, including wound care, catheter insertion, and monitoring vital signs.
• Assist in developing and implementing individualized care plans in collaboration with the patient, family, and healthcare team.
• Monitor and document patient progress, response to treatments, and any changes in condition, reporting significant findings.
• Provide education and support to patients and their families on disease management, medication use, and home care techniques.
• Assist with patient mobility, transfers, and activities of daily living as needed, ensuring safety and comfort.
• Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
• Collaborate with LPNs, physicians, and other healthcare professionals to ensure coordinated and comprehensive care.
• Respond to patient needs and changes in condition promptly, providing appropriate interventions and support.
• Adhere to infection control and safety procedures to protect patients and staff.
• Perform all other duties assigned.
