Humana Inc. through CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to provide in-home therapy under the supervision of a Physical Therapist.

You will help patients regain strength, mobility, and independence by delivering personalized care focused on functional improvement in the patient's home. Responsibilities include executing the prescribed PT plan, performing non-complex manual therapies, documenting records, and coordinating with the team.

Qualifications

• Current state license to practice Physical Therapy is required.
• CPR certification required.
• Experience collaborating with a team of healthcare professionals is required.
• Knowledge of geriatric population changes is required.
• Read, write, and speak fluent English.

Responsabilités

• Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
• Administer non-complex manual therapies, therapeutic exercises, and modalities as directed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
