• Licensed PTA in the state of practice or eligible to obtain license
• Current CPR certification
• Proficient with Microsoft Office and basic computer skills

Responsibilities

• Deliver individualized treatment plans established by supervising Physical Therapist
• Instruct and motivate patients through therapeutic exercises and functional activities
• Document visits accurately and on time in FOX’s EMR system
• Coordinate care with patients, POAs, and community referral sources
