Job summary

Mind & Mobility Home Care is seeking a licensed Physical Therapist Assistant to deliver in-home Part B therapy to Medicare patients in Davie, Miramar and Broward County under supervision of a licensed Physical Therapist. The role involves implementing treatment plans, improving mobility and function, educating patients and caregivers, and documenting progress according to Medicare Part B guidelines.

Requires PTA license, driver’s license, CPR, and the ability to work independently with flexible

Qualifications

• Active PTA license in state of practice
• Experience in home health or outpatient therapy preferred
• Knowledge of Medicare Part B documentation/regulations
• Strong patient care, treatment implementation and communication skills
• Ability to follow treatment plans and report progress
• Reliable transportation and a valid driver's license
• CPR and other relevant certifications as required

Responsibilities

• Assist in implementing individualized treatment plans
• Provide skilled therapy to improve mobility, strength, balance
• Educate patients and caregivers on exercises and safety
• Document patient progress per Medicare Part B guidelines
• Communicate with supervising PT and therapy team
• Monitor patient response and report progress or concerns
• Maintain a safe, professional environment during home visits
