Job summary

Mind & Mobility Home Care seeks a Physical Therapist Assistant to provide in-home therapy for Medicare Part B patients under the supervision of a licensed Physical Therapist. The role focuses on implementing individualized plans to improve mobility and function during home visits.

The position requires strong patient care, communication, and the ability to work independently with a flexible schedule. Prior home health or outpatient therapy experience is preferred, and CPR or equivalent

Qualifications

• Active PTA license in the state of practice.
• Experience in home health or outpatient therapy preferred.
• Knowledge of Medicare Part B documentation and regulations.
• Strong patient care, treatment implementation, and communication skills.
• Ability to follow treatment plans accurately and report patient progress.
• Reliable transportation and a valid driver’s license.
• Comfortable working independently and managing a flexible, home-based schedule.
• CPR and other relevant certifications as required.

Responsibilities

• Assist in implementing individualized treatment plans developed by the supervising Physical Therapist.
• Provide skilled therapy interventions to improve mobility, strength, balance, and function.
• Educate patients and caregivers on exercises, safety techniques, and fall prevention strategies.
• Document patient progress accurately and in compliance with Medicare Part B guidelines.
• Communicate effectively with the supervising Physical Therapist and the interdisciplinary home health/therapy team.
• Monitor patient response to therapy and report progress or concerns to the Physical Therapist.
• Maintain a safe and professional environment during home visits.
