Job summary

FOX Rehabilitation in Grinnell, IA is looking for licensed Physical Therapist Assistants to provide one-on-one, in-home and senior living care under a supervising Physical Therapist. Qualifications

• Valid Physical Therapist Assistant license in the state of practice.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.

Responsibilities

• Deliver individualized treatment plans established by supervising PT, adjusting activities based on progress.
• Instruct and safeguard patients through therapeutic exercises and functional activities, monitoring progress.
• Document visits accurately within FOX’s EMR and follow clinical documentation standards.
• Coordinate care with patients, POAs, and community referral sources.
