Job summary

FOX Rehabilitation is seeking licensed Physical Therapist Assistants to provide in‑home care in Grinnell, IA. You will work closely with a supervising PT to implement individualized plans, guide patients through exercises, and support mobility and confidence in the home setting.

You’ll join an interdisciplinary team with mentorship and growth opportunities, delivering compassionate, hands‑on care to older adults in their living environments.

Qualifications

• Active Physical Therapist Assistant license for practicing states.
• Graduate of an accredited PTA program and CPR certification.
• Proficient with standard office software and EMR systems.
• Ability to work closely with supervising PT in home-based care.

Responsibilities

• Deliver individualized treatment plans under supervising PT.
• Guide patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX’s EMR system.
• Coordinate with POAs and referral sources for coverage.
