Job summary

FOX Rehabilitation is hiring a licensed Physical Therapist Assistant to provide in‑home and senior living care in Fort Dodge, IA, working under a supervising Physical Therapist. You will implement individualized treatment plans, guide patients through exercises, and document progress in FOX’s EMR system.

You’ll join an interdisciplinary team, receive mentorship, and grow your caseload and clinical skills within a supportive setting focused on independence and quality of life for older adults.

Qualifications

• Must hold a valid Physical Therapist Assistant license in the practiced state(s).
• Current CPR certification is required.
• Proficiency with basic computer applications, including MS Office.

Responsibilities

• Deliver individualized treatment plans under supervising PT, adjusting as needed.
• Instruct and guide patients through therapeutic exercises and functional activities.
• Accurately document visits in FOX EMR per Clinical Documentation standards.
• Coordinate with patients, PoAs, and community sources to ensure coverage when needed.
