Job summary

Guardian Angel Home Care is seeking compassionate Physical Therapist Assistants to join our Home Health team. The PTA works under the supervision of a licensed Physical Therapist to provide prescribed therapy to patients in their homes, helping improve mobility and quality of life.

Responsibilities include performing treatment programs, documenting progress, collaborating with the care team, and assisting with discharge planning.

Qualifications

• Graduate of a Physical Therapist Assistant program accredited by the APTA and/or AMA.
• Current PTA license in the state of employment.
• Minimum one year of clinical experience preferred.
• Home Health experience preferred but not required.

Responsibilities

• Conduct medically prescribed therapy programs involving exercise, massage, heat, water, light, and electrical modalities.
• Record and report patient progress and response to treatment.
• Collaborate with supervising Physical Therapists and interdisciplinary care teams.
• Prepare and maintain accurate clinical documentation.
• Assist with discharge planning in coordination with the therapy case manager.
