Job summary

Guardian Angel Home Care is seeking a compassionate Physical Therapist Assistant to provide in-home therapy under the supervision of a licensed Physical Therapist. You will help improve mobility, strength, and independence for patients in their homes.

The role requires PTA licensure and relevant clinical experience; Home Health experience is preferred. Field-based work across service areas with opportunities for growth and competitive compensation.

Qualifications

• Graduate of a Physical Therapist Assistant program accredited by the APTA and/or AMA.
• Current PTA license in the state of employment.
• Minimum one year of recent clinical experience preferred (Home Health experience preferred).
• Home Health experience preferred but not required.

Responsibilities

• Conduct medically prescribed therapy programs involving exercise, massage, heat, water, light, and electrical modalities.
• Record and report patient progress and response to treatment.
• Collaborate with supervising Physical Therapists and interdisciplinary care teams.
• Prepare and maintain accurate clinical documentation.
• Assist with discharge planning in coordination with the therapy case manager.
