Job summary

FOX Rehabilitation in Bordentown, NJ is seeking licensed Physical Therapist Assistants to deliver one-on-one, in-home care as part of our geriatric House Calls model. You will work closely with a supervising PT to guide patients through individualized treatment plans and functional activities.

Qualifications

• Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR certification
• Basic computer skills; proficient in Microsoft Office

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist
• Instruct, motivate, and safeguard patients through therapeutic exercises and activities
• Document visits accurately in FOX’s EMR system
• Coordinate with patients, POAs, and referral sources
