Job summary
FOX Rehabilitation in Morganville, NJ seeks licensed Physical Therapist Assistants to provide in-home, one-on-one care under a supervising Physical Therapist. You will deliver individualized treatment plans and guide patients through therapeutic exercises and functional activities in FOX’s Geriatric House Calls model, delivered where patients live.
This role offers mentorship from a supervising PT, growth within an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists, and a clear
Qualifications
- Licensed Physical Therapist Assistant in the state of practice or eligibility to obtain license.
- Current CPR certification is required.
- Basic computer skills; proficient in Microsoft Office.
Responsibilities
- Deliver individualized treatment plans established by supervising PT, adjusting activities based on patient response and progress.
- Instruct and guide patients through therapeutic exercises and functional activities; monitor progress.
- Document visits accurately in FOX’s EMR system per Clinical Documentation standards.
- Coordinate with patients, POAs, and referral sources and assist with coverage when needed.
