Job summary

Harris Home Health, part of the LHC Group and aligned with Optum family, seeks a Physical Therapist to perform patient assessments, develop and implement plans of care, and provide therapeutic interventions in patients’ homes.

You will evaluate progress, educate patients and families, and document outcomes while working with a multidisciplinary team in a supportive, mission-driven environment.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Driver’s license and access to a dependable vehicle, or public transportation.
• CPR certification (preferred within 90 days of hire).

Responsibilities

• Evaluate the patient's functional status and physical therapy needs; assist in developing the therapy plan of care.
• Provide PT services within scope of practice and plan of care, in coordination with the health care team.
• Observe and report patient responses to treatment to supervisor or physician.
• Treat patients to relieve pain and restore or maintain function according to standards.
• Instruct patient and family about plan of care, limitations, and progress towards goals.
• Visit patients according to Plan of Treatment and submit progress notes per policy.
• Complete evaluations and develop the PT plan of care within state guidelines.
• Report outcomes and anticipated frequency of care.
• Function in diverse home environments regardless of patient demographics.
