Always Best Care in O’ahu, HI is seeking licensed Physical Therapists and Physical Therapist Assistants for in-home care on a per diem basis. The role includes in-home visits, patient evaluations, and fall risk assessments to help seniors stay independent at home.

Flexible scheduling with per diem opportunities; reliable transportation is required. Current Hawaii license and 1+ year of clinical experience (home health preferred) are expected, along with strong documentation and communication

Qualifications

• Current Hawaii license as PT or PTA.
• 1+ year of clinical experience (home health preferred).
• Strong documentation and communication skills.
• Valid driver's license and reliable transportation.

Responsabilités

• Provide in-home physical therapy visits.
• Complete evaluations, reassessments, and fall risk assessments.
• Document accurately and communicate progress with the care team.
