Job summary

Always Best Care Senior Services is hiring licensed Physical Therapists and Physical Therapist Assistants to provide in-home visits on O’ahu, HI. Per diem, flexible scheduling, and professional autonomy are offered to support seniors’ mobility and safety at home.

Responsibilities include evaluating and reassessing patients, addressing fall risk, and accurately documenting progress for the care team. Valid Hawaii PT/PTA license and transportation are required.

Qualifications

• Current Hawaii license as PT or PTA.
• 1+ year of clinical experience (home health preferred).
• Strong documentation and communication skills.

Responsibilities

• Provide in-home physical therapy visits.
• Complete evaluations, reassessments, and fall risk assessments.
• Document accurately and communicate progress with the care team.
