Job summary

Avid Health at Home, LLC is seeking a PRN Physical Therapist to provide skilled physical therapy services in patients' homes under physician orders and agency policies. The PT will assess patients, formulate individualized treatment plans, implement interventions, and collaborate with the interdisciplinary team to maximize independence and safety.

The role supports flexible scheduling with PRN availability and is grounded in Medicare/HIPAA compliance and quality patient care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, unrestricted Physical Therapist license in the state of practice.
• Current CPR/BLS certification.

Responsibilities

• Perform comprehensive physical therapy evaluations and determine functional status in patients' homes.
• Develop individualized plans of care based on physician orders and goals.
• Provide skilled PT interventions including therapeutic exercise, gait and balance training, and fall prevention.
• Educate patients and families on home exercise programs and safety techniques.
• Document evaluations, visits, and progress notes in compliance with policies.
