Responsibilities

• Develop and implement personalized care plans focused on mobility, safety, and activities of daily living (ADLs).
• Provide hands-on therapeutic interventions and monitor patient progress.
• Educate clients, families, and caregivers on safe transfer techniques and home modifications.
• Maintain clear, reliable communication with families and the clinical coordination team.
